Administrative Forms

| Emergency Contact Information* | Emergency contact information to help us reach you in the event of a problem. |
| Health* | Health information required by the state. Must be completed by child’s health care provider. |
| Child Care Immunization Record* | Medical information required by the state. |
| Student Information Form | Basic information about the student and their family that helps us understand them and their unique needs. |
| Financial Contract & Permission* | Financial contract portion of the form outlines guidelines for tuition payment. Permission portion of the form is where you indicate activities you allow for your child. |
| Scholarship Application | Application for financial assistance. |
| Snuggle Pack Contract | Guidelines and signature form required for your child to participate in the Snuggle Pack program. |
| Snuggle Pack Sponsorship | Support the Snuggle Pack family activity and literacy support program. |
| Background Check | State requirement for all those who wish to volunteer at the school. |

Field Trip Permission Form
Each field trip has its own permission form. The form for the currently upcoming field trip is available for download here. The form posted here is updated as each trip approaches. You can also review the field trips page for a complete listing of the trips planned for the current academic year.
Accident and Emergency Policy
This is the Sunshine Preschool Accident and Emergency Policy. Parents also receive a copy of this policy when they complete the Emergency Contact Form.
